Advocacy and Family Assistance

Mid-West Family and Community Engagement (FACE) Center supports families and other stakeholders in understanding and navigating the special education system, for ages birth to 22. We provide technical assistance and learning opportunities for families and community members related to the Committee on Special Education and the Committee on Preschool Special Education, the Individualized Education Program, transitions along the special education continuum from Early Intervention through Post-Secondary Transition, navigating adult and community services for individuals with disabilities, and other topics related to special education.

Person Centered Services is a Health Home/Care Coordination Organization for people with intellectual and developmental disabilities. At the heart of Care Coordination are connections and referrals that provide positive outcomes. Person Centered Services’ Care Coordinators work continuously with individuals, families, community providers and organizations to make sure the dots are connected between all aspects of a person’s care. Individuals with intellectual and developmental disabilities may be eligible for supports and services from New York State’s Office for People with Developmental Disabilities, and therefore eligible for Care Coordination.

As a Care Coordination Organization, or CCO, Prime Care Coordination provides care management services for adults and children with intellectual and developmental disabilities. If individuals seeking care management services do not have eligibility through New York’s Office for People with Developmental Disabilities (OPWDD), then Prime Care’s enrollment team can assist with the eligibility process and the OPWDD Front Door process. Enrolling in Prime Care is a two-step process. Simply call 1-844-347-3168, select “Option 1” and begin enrollment. Prime Care is unique in that we offer the services of a Student and Family Educational Specialist. This person assists children in Early Intervention through the transitioning process into adult services.

Education and Recreation

Autism Up supports individuals with autism, and their families, by expanding and enhancing opportunities to improve quality of life. Parent perspective steers the direction of the organization, with the fiduciary guidance of our Board of Directors. Together, we are providing help for today, and hope for tomorrow.

Spotted Rabbit Studio offers licensed creative arts therapy and visual arts classes to individuals ages 5+ of all abilities. Our art therapists specialize in developmental disabilities and neurodivergence (autism and ADHD), as well as complex trauma and LGBTQ+ issues.

Health, Wellness and Medical

The CUNY Graduate School of Public Health & Health Policy for Implementation Science in Population Health (ISPH) brings together multidisciplinary teams of researchers to conduct impactful, cutting edge research on population health and related outcomes, including programmatic and service delivery implementation, and clinical interventions. We provide research and training opportunities for faculty, students, post-doctoral fellows, interns, and visiting scholars. The Institute also curates and hosts themed special events, such as symposia and seminars. STEPS2 is recruiting young adults with IDD to take classes on sex education or healthy behaviors for a randomized controlled trial.

Multiple Services

The Adult and Career Continuing Education Services - Vocational Rehabilitation's (ACCES-VR) mission is to assist students and adults with disabilities in achieving and maintaining employment and supporting independent living through pre-employment transition services and other supports. ACCES-VR offers different entry points for students starting at the age of 14.
